앞으로 제가 일하면서 겪은 일과 공부하는 내용을 공유하고자 합니다><

 

#1 CASE1

첫 번째 사례는 1990년 유방암에 확진되었다가 간, 폐, 뇌까지 전이된 환자 이야기입니다. 

의사는 개두술을 통해 뇌암을 제거했고 두개골을 고정장치로 고정하고 환자는 후속치료로 방사선 치료를 받았습니다. 

1년 넘게 환자는 재발 없이  지내다가 3주 전 환자가 두피 쪽에 통증을 호소하기 시작합니다. 

의사는 두피를 다시 열어 통증의 원인을 찾기로 합니다. 

 

[대화문] 

 

Dr: Breast cancer in 1990.

      Liver mass, breast, and then primary lung also?

Dr: Show me the bump

Patient: it radiates

Dr: You got radiation here?

Patient: I did. that's why my hair fell out.

Dr. Well, your hair looks great, so that's why I was asking.

Patient: We're doing scalp exploration. resection of the mass.

I love when it said on the form that I originally that is said "hardware removal"

Dr: Okay, you've signed a lot of forms this morning. Welcome to the bureaucracy.

Patient: So can I get the screw and the plate to make

Dr: So we're going to go in and open her scalp. Um, i did a brain surgery on her for metastasis to the brain,

which she's doing terrific from. She had radiation and has been disease-free for over a year. 

Interestingly, about three weeks ago, she developed real pain in the scalp area.

And so in a patient who's had a history of cancer, we worry about metastasis.

Or in a patient that's had a history of surgery, we worry about something in the fixation system of the skull back onto her natural skull. And that may be a plate that's come loose or is irritating the scalp. 

So it's my job to figure it out for sure. 

Dr: Keep our fingers crossed that it's not cancer, and you're home. 

Patient: I get nice staples in my head. 

Dr. You get staples in your head and good drugs when you wake up

    I'll see you inside.  Nothing to worry about. 

 

전이라는 표현은

metastasis  입니다. 

영문을 찾아보면 the development of secondary malignant growths at a distance from a primary site of cancer 입니다. 

즉 원래 발생한 암 위치에서 벗어난 곳으로 암이 이동했다는 뜻 입니다.

병이 없다는 disease-free  라는 표현을 썼습니다! 

병력의 경우는 history 라는 말을 씁니다! 

암을 앓았던 환자 ->  a patient who's had a history of cancer,

수술을 했던 환자-> patient that's had a history of surgery 

 

의사가 추정하는 환자의 통증을 야기할 수 있는 두 가지의 요소는 다음과 같습니다.

 

1) 암 전이 [metastasis]

2) 두개골 고정장치 판이 헐거워져서 두피를 자극 [a plate that's come loose or is irritating the scalp. ]

 

 

보통 좋은 일이 있길 바란다고 할 때 Keep our fingers crossed 를 씁니다. 

의사는 암이 아니길 희망한다는 희망의 메시지를 전달합니다. 

Keep our fingers crossed that it's not cancer!
