오늘은 소아청소년내과 통역 시나리오를 공부해 보도록 하겠습니다. 

제가 통역 다녀온 내용을 기반으로 시나리오를 작성해 보았습니다!

 

 

 

의사: 애기 컨디션 좋은가요?

아버지: 네 잘 지냅니다. 그런데 아이가 대변을 잘 안 봅니다. 토요일 전까지는 하루에 한 번 대변을 보고 토요일에 대변을 두 번 보더니 일요일부터 오늘(화요일)까지 대변일 한 번도 보지 않았습니다. 큰 문제가 있을까요?

의사: 큰 문제는 없지만, 대변을 3~4일에 한 번씩 보게 되면 대변이 딱딱해질 수 있습니다. 괜찮다면 관장약을 조금 써보도록 하겠습니다.

아버지: 그렇게 해주세요

의사: 수유는 어떻게 하고 있나요?

아버지: 낮에는 모유수유를 3시간씩 하고 저녁에는 4시간 마다 먹습니다. 그런데 잘 먹지 않아서 보충 수유를 합니다.

의사: 모유는 잘 나오나요? 아이가 젖꼭지 빠는데 어려움을 겪는건가요?

아버지: 모유는 잘 나오는데 아이가 젖꼭지 빠는데 어려워합니다. 분유젖병으로는 잘 먹습니다.

의사: 아이가 젖꼭지 빠는데 어려움을 겪으면 모유수유 시간이 길어지고 아이가 쉽게 지키기 때문에 잘 먹지 않게 됩니다. 모유를 충분히 짜서 젖병에 넣어서 주는 것이 좋습니다. 물론 가장 좋은 방법은 직접 모유수유입니다. 아이가 엄마의 체온을 느낄 수 있어서 정서적으로 안정되고 어머니의 항체가 아이에게 전달됩니다. 하지만 아이가 작게 태어났기 때문에 많이 먹이는 게 정말 중요합니다. 자주자주 수유를 해주어서 충분히 먹을 수 있도록 해주세요.

아버지: 네 알겠습니다.

의사: 혹시 비타민 D 먹이고 있을까요?

아버지: 아니요

의사 비타민 D 시럽을 먹이는 게 좋습니다. 유일하게 아이에게 먹일 수 있는 영양제입니다. 처방아버지: 처방해주 실 수 있나요?

의사: 처방약은 너무 비싸기 때문에 약국에서 사는 것을 권장드립니다. 보통 모유수유만 하는 아이에게는 철분이 함유된 비타민D를 권고합니다. 보충 수유를 하는 아이에게는 철분이 함유되지 않은 비타민 D를 권합니다. 약국에서 사시면 됩니다.

아버지: 감사합니다. 혹시 종이에다 써줄 수 있나요?

통역사: 알겠습니다. 여기 썼습니다. 전화번호도 남겼습니다. 약사가 이해를 못하면 이 전화번호를 남겼으니 이쪽으로 전화하라고 해주세요

의사: 아이 잘 먹이고 예방접종도 잘 하기 바랍니다.

의사: 아이 접종은 어떻게 하지요? 다음 예방 접종이 언제이지요? 저희가 7월 말에 인도에 가서요.

의사: 오늘 온 김에 BCG 접종 하고 가세요. 7월 11일에 B형간염 2차 맞으면 되겠습니다. 관장도 하고 가시면 됩니다.

아버지: 알겠습니다.

의사: 통역사님 수고 많았습니다. 감사합니다.

 

 

 

Dr: How has she been, is she feeling okay?

Father: Yes, she has been feeling okay. However, my child is not having regular bowel movements. Until Saturday, she had one bowel movement per day, but on Saturday, she had two bowel movements. Since Sunday until today (Tuesday), she hasn't had any bowel movements. Is there a big problem?

Dr: There may not be a major problem, but if your child has bowel movements every 3 to 4 days, her stool can become hard. If it's okay, I would suggest trying a mild laxative.

Father: Please go ahead and try using a mild laxative in that case.

Dr: How are you currently feeding her?

Father: During the day, we breastfeed her every 3 hours, and in the evening, we feed her every 4 hours. Additionally, we supplement with formula milk because she is not eating well.

Dr: Is her breast milk coming out well? Or Is your baby having difficulty latching onto the nipple?

Father: The breast milk is coming out well, but the baby is having difficulty latching onto the nipple. She feeds well with a bottle of formula milk.

Dr: If the baby is having difficulty latching onto the nipple, the feeding time may become longer, and makes her very tired, making it difficult to feed well. Therefore, it is advisable to express milk from her breast and offer it to the baby in a bottle. Of course, direct breastfeeding is the best method if possible. It provides emotional stability for the baby by allowing them to feel the mother's body warmth, and it also allows the transfer of maternal antibodies. However, since the baby was born small, it is important to feed her enough.  Please try to breastfeed frequently to ensure she can have enough milk.

 

Expressing milk 

means squeezing milk out of your breast so you can store it and feed it to your baby later.

 

Father: I got it.

Dr: Are you giving her Vitamin D?

Father: No.

Dr: It is recommended to give your baby Vitamin D syrup. It is the only nutritional supplement that can be given to infants.

Father: Can I get a prescription for Vitamin D?

Dr: Because prescription medication can be expensive, I recommend purchasing it from a pharmacy. For babies who are exclusively breastfed, it is recommended to use Vitamin D with iron. For babies who receive formula supplementation, Vitamin D without iron is recommended. You can buy it at a pharmacy.

Father: Thank you. Can you write it down on a piece of paper, please?

Interpreter: Certainly. I have written it down for you. I also left my phone number here. If the pharmacist doesn't understand, please ask them to call this number for further assistance.

Dr: Please feed your child well and make sure to get her vaccinated as scheduled.

Father: When is the next vaccination scheduled? We are going to India at the end of July.

Dr: She can be vaccinated against BCG while she’s here today. She can get the second dose of Hepatitis B shot on July 11th. Are you okay with that?

Father: That’s fine.
